چکیده

In the article ‘Time to refine N2 staging’ [1], Matsunaga et al. investigated the relationship between the prognosis and the location of the primary tumour and nodes (LN) involved. They suggested a classification of these patients to either cN2α, which consisted of an upper lobe tumour with clinical positive ipsilateral upper mediastinal LNs (station: 2R and 4R on the right and 2L, 4L, 5 and 6 on the left), or cN2β, which was a lower lobe tumour with clinical positive ipsilateral mediastinal LNs (station: 7, 8 and 9 on both sides) [1]. They found that survival was better in the cN2α group regardless of the number of LN stations involved. The following points need to be considered:

Surgical Management of Stage IIIA Non-Small Cell Lung Cancer

According to the eighth edition of the tumor-node-metastasis classification, stage III non-small cell lung cancer is subdivided into stages IIIA, IIIB, and IIIC. They represent a heterogeneous group of bronchogenic carcinomas with locoregional involvement by extension of the primary tumor and/or ipsilateral or contralateral lymph node involvement. Surgical indications have not been definitely e...

Recursive Partitioning Analysis of Mediastinal N2 Lymph Node Involvement with Selected Biological Markers in Operable Non-small Cell Lung Cancer: A Correlative Study

BACKGROUND Expressions of various biomarkers in non-small cell lung cancer (NSCLC) have been linked with the prognosis and involvement of mediastinal lymph nodes. METHODS In this study, we utilized recursive partitioning analysis (RPA) by using P53, c-erb-B2, and P-glycoprotein (PGP) expressions evaluated by immunohistochemistry to estimate retrospectively the likelihood of the occult N2 medi...

Does Opium Dependency Affect the Pattern of Involvement in Laryngeal Cancer?

Introduction: Laryngeal cancer is the second most common cancer involving the respiratory tract, second only to lung cancer. Previous studies have shown opium dependency to be a possible risk factor for laryngeal cancer. Opium consumption is a major problem in some parts of the world. The aim of this study was to investigate the effect of opium consumption on the pattern of involvement in laryn...

Clinical predictors of N2 disease in non-small cell lung cancer.

OBJECTIVES To identify clinical or radiologic predictors of mediastinal lymph node involvement in patients with non-small cell lung cancer, and to define the indications of preoperative mediastinoscopy. METHODS From August 1992 through April 1997, 387 patients with lung cancer (290 adenocarcinoma and 97 squamous cell carcinoma) underwent surgical resection. We retrospectively measured all med...

Long-term survival after complete mediastinal lymph node resection and lobectomy in patients with bulky N2 non-small cell lung cancer.

BACKGROUND At Juntendo University Hospital, Japan, two patients with complete resection of bulky N2 non-small cell lung cancer (NSCLC) has survived over the long term. Therefore, we decided to retrospectively study patients who also had a complete resection of the tumor including the "bulky" superior mediastinal node for the purpose of reviewing covariates that might be related to the favorable...
